MyHealthPass is an authorized direct to consumer reseller partner for Teladoc, Inc. MyHealthPass/Teladoc does not replace the primary care physician. MyHealthPass/Teladoc does not guarantee that a prescription will be written. MyHealthPass/Teladoc operates subject to state regulation and may not be available in certain states. MyHealthPass/Teladoc does not prescribe DEA controlled substances, non-therapeutic drugs and certain other drugs which may be harmful because of their potential for abuse. MyHealthPass/Teladoc physicians reserve the right to deny care for potential misuse of services. MyHealthPass website allows for individuals to enroll in our non-insurance healthcare benefit. By executing a transaction through the MyHealthPass website, or any of its affiliated websites, or by phone through a MyHealthPass member services representative, you agree to be billed on a recurring monthly basis via the credit card or bank account billing information that you have provided for the amount of the specified service in which you have enrolled. Recurring monthly payments will continue until such time as you cancel your account either in writing by submitting a notice of cancellation by mail at MyHealthPass, attention: customer care center, 15280 Addison Rd., ste.250, Addison, TX 75001; or by contacting by phone 855-212-6020 Monday through Friday from 9AM TO 5PM CST.
MyHealthPass provides all members with a 30 day money back guarantee. You MUST cancel your MyHealthPass membership within thirty (30) calendar days of your initial purchase in order to receive a refund.
Any cancellations after thirty (30) calendar days of purchase will not receive a refund.
By executing a purchase transaction via MyHealthPass (et al) you agree to the terms of this REFUND/PURCHASE POLICY without exception, and the TERMS OF USE detailed below.
